APPOINTMENT, PAYMENT and INSURANCE INFO

Appointment Information:

Your initial/first appointment with your provider will be about 45-50 minutes in length. After your initial appointment, we will establish any necessary follow-up appointments, as requested/required by your provider. This may involve setting up a "standing" appointment (same time and day of each week.) Follow-up appointments are also 45-50 minutes long.

If you cannot make it to your appointment for a foreseen reason (e.g. other previously scheduled appointment, work meetings, no transportation, etc.) you must call us with 24-hour advanced notice to avoid a No-Show Fee or Late Cancellation Fee. Unavoidable or last-minute events (e.g. flat tire, child getting sick at school, etc.) MUST still be called in to avoid being assessed a fee.

To CANCEL your appointment, please CALL our office, do not send email to the provider.

We ask that you please do NOT come to the appointment if you are ill. This helps to prevent the spread of any contagions, and keeps our providers healthy and available to conduct services.

We also ask that, if at all possible, you do NOT bring your small children with you to your appointment. If you are not able to make other childcare arrangements, please call us ahead of time to see if we can reschedule your appointment, or clear it with your provider for you to bring your child/children. Please keep in mind that your children will probably go into the session with you, as our staff are not childcare providers. Having your small children in session with you may cause distractions, or prevent open discussion with your provider, and thus be counter-productive to your treatment.

Payment Information

We accept cash, checks, money orders, and credit cards.          .

If you have a CO-PAY, we require it at time of service.  If you have a DEDUCTIBLE and/or CO-INSURANCE, we will submit the claim for service to your insurance company, and when we receive the Coordination of Benefits/Explanation of Benefits statement (either with or without payment), we will bill you for your remittance.

 The billing statements go out during the first week of each month, and payment is due within 30 days from date of statement.  You can mail your payment, bring cash, pay at your next appointment (within the 30 day period), or call us to take a payment by credit card (we do NOT store/save card numbers/information)
